How To Apply

For individuals new to Thames Valley Police, please contact the Federation office as there may be an introductory offer available to you.

Please note that if to wish to apply for the Core Scheme (reduced cover) please see details on our ‘Group Insurance Core Scheme’ page.

New Recruits

• New recruits are given an input by the Federation during their first few weeks of training. They will be given all information and opportunity to sign up to the Group Insurance Scheme during this input. This will include partner cover and top up covers

All Application Forms can be downloaded in the ‘Downloads’ side bar section. Description of forms is as follows:

• 01 – TVP Membership Package App – This includes membership to the Federation & Group Insurance Scheme and cover for Partners. You only need to complete what is relevant
• 02 – TVPGIS Fully Underwritten App  – This form is to be used if you cannot complete the health declaration on form 01 and you have medical conditions or history that the underwriter will need to assess. This form is for membership to the Group Insurance Scheme and partner cover
• 03 – Beneficiary Form Serving – This form is for Members of the Group Insurance Scheme to change or update details of their nominated beneficiary
• 04 – Partner Beneficiary Form – This form is for Partners of the Group Insurance Scheme to change or update details of their nominated beneficiary
• 05 – Core Scheme Application – Please see our Group Insurance Core Scheme for more details
• 06 – Specials Core Application – This is the National Special Constable scheme, all Special Constables should complete this application form.
• 07 – PW Top Up Cover – If you want additional Life/Critical Illness cover
• 08 – PW Top Up Cover Full underwriting – Additional Life/Critical illness cover if medical conditions exist.
• 09 – SEERPIC App – For SEERPIC members only, please complete this form if you have no medical conditions to declare.
• 10 – SEERPIC Fully Underwritten – For SEERPIC members only, please complete this form if you have medical conditions to declare.
• 11 – SCAS App – For SCAS members only, please complete this form if you have no medical conditions to declare.
• 12 – SCAS Fully Underwritten – For SCAS members only, please complete this form if you have medical conditions to declare.
• 13 – SCAS Fully Underwritten – For SCAS members only, please complete this form to join the Core Scheme.

Serving Officers / Transferees / Late Entrants / Staff

• Please complete Form 01 if you have no medical conditions to declare and Form 02 if you need to notify the insurer of any medical conditions.
• Transferees, please ensure that you provide your payslip from your last force showing membership of a similar scheme within 3 months of joining This enables you to be accepted in the scheme without medical underwriting. If you cannot provide the payslip or were not a member of a scheme previously, please complete an application including a signed health declaration or full medical underwriting
• Late entrants to the scheme who are not Transferees and all new Staff members will get 3 months of free cover.

Partners of Above

• Please complete Form 01 if you have no medical conditions and Form 02 if you need to notify us of medical conditions

Top Up Cover (Life and Critical Illness) – Also available for Partners

• Please complete Form 07 if you have no medical conditions and Form 08 if you need to notify us of medical conditions

Retired Officers

• Upon retirement your membership will automatically move to the Retired Scheme, unless you elect not to continue with cover, and cover is then applicable until your 80^th birthday. Unfortunately we are unable to accept new applications from Retired Officers. It is not possible to upgrade the cover on the Retired Scheme.
