If you are unfortunate enough to be diagnosed with a Critical Illnesses please contact us for a claim form or download the relevant claim form from the menu at the side of this page. If you are claiming for a Terminal diagnosis advance payment, please use the TPA claim form.
Cover for all critical illnesses is subject to the relevant condition being assessed to the level of severity contained within the policy.
