Claims in respect of Life Insurance will be made on your behalf through the Federation Office. Once the Trust has been informed of the death of a member, a Representative will make contact with the family and request the appropriate documentation. Please do not complete a Life Claim form, this should only be completed by Trust Admin Staff. Please contact enquiries@tv.polfed.org to commence a claim.
An original death certificate or an original coroners certificate will be required before a claim can be processed.
Please complete the Critical Illness Claim Form document in the ‘Downloads’ side bar section, or request one is sent to you by the Federation office. Completed forms should be emailed where possible to: enquiries@tv.polfed.org or alternatively posted to the Federation office. Please do not send the forms direct to Philip Williams & Company, they must come to the Federation Office first.

A Initial Sick Pay Claim Form form will then be sent to you for completion, when submitting the claim form the insurer will also require a copy of the letter advising you of your reduction in pay, a copy of your last payslip indicating that you were being fully paid, and a copy of your first payslip indicting your reduction in pay. Complete the Hospital Benefit Claim Form form which can be downloaded from the side bar menu and send this should be submitted to the insurer together with a hospital admission and discharge certificate.

Complete the Dental Benefit Claim Form form which can be downloaded from the side bar menu and send Philip Williams, this should be submitted to the insurer together with receipts for the treatment which was carried out.

Complete the On Duty Assault Claim Form form which can be downloaded from the side bar menu and send, this should be submitted via the Federation Office together with supporting medical certificates. The claim form will need to be signed by a senior officer.
If a member has to stay in a police convalescent home on the recommendation of a registered medical practitioner in respect of accident or illness, the amount shown in the benefits table will be paid. Please complete the Convalescent Benefit Claim Form which can be downloaded from the side bar menu and submit to Federation Office.
